Translation, cultural refinement, content re-validation & application of OTAS and METEOR instruments in Turkish operating rooms: An observational study

Objective: The aim of this study was to translate, culturally adapt, content validate, and test the reliability of The Observational Teamwork Assessment for Surgery (OTAS) and the Metric for Evaluating Task Execution in the Operating Room (METEOR) in Turkish operating rooms (ORs). Methods: This study used an observational design. After translation of OTAS and METEOR in Turkish, 22 OR experts rated the degree to which each OTAS and METEOR items contributes to teamwork, patient safety, and efficiency. Afterward, real-time observations were conducted using both tools in 20 elective general surgeries to evaluate feasibility of use and reliability of scoring. Kappa and intraclass correlation coefficients were used to evaluate reliability. Results: OR experts deemed all OTAS and METEOR items to be content valid. Perfect, very good, and good interobserver agreement were found for 75.4% of OTAS and 86.25% of METEOR items (kappa coefficient ? 0.61). Interrater reliability of OTAS ratings ranged from 0.43 to 0.80. The surgical team's OTAS scores were significantly lower than the nursing and anesthetic teams, while the anesthetic team's scores were significantly higher than the nursing team's scores (p < 0.05). The preoperative OTAS scores were statistically higher than the intraoperative and postoperative scores, while the intraoperative scores were significantly higher than the postoperative scores (p < 0.05). Conclusion: OTAS and METEOR are content valid tools that are feasible to use reliably in Turkish ORs. © 2020 Elsevier Inc.
