İRRİTABL BAĞIRSAK SENDROMLU (İBS) HASTALARDA SERUM YKL-40 VE NETRİN-1 SEVİYELERİNİN DEĞERLENDİRİLMESİ

Özet
Dünyada yaygın görülen fonksiyonel gastrointestinal bir bozukluk olan İBS, yaşam kalitesini olumsuz etkilemekte ve yüksek oranda sağlık harcamalarına neden olmaktadır. Patofizyolojisi tam olarak bilinmeyen bu hastalıkla ilgili çeşitli hipotezler öne sürülmektedir. Son zamanlarda yapılan birçok çalışmada düşük düzeydeki mukozal inflamasyon ve immun sistem disfonksiyonun İBS patofizyolojisindeki rolü vurgulanmaktadır. İBS hastalarında pro-inflamatuvar ve anti-inflamatuvar sitokinlerde dengesizlik olduğu gösterilmiştir. İnflamasyon ve enfeksiyona katılan çeşitli hücrelerden sekrete edilen YKL-40 proteininin immun regülasyonda rol oynadığı ileri sürülmektedir. Netrin-1’ in inflamasyon aracılı doku hasarını azalttığı ve anti-inflamatuvar etkiye sahip olduğu gösterilmiştir. Çalışmamızda immun aktivasyon ve inflamasyon ile ilişkili olduğu düşünülen YKL-40 ve netrin-1 seviyelerinin İBS patofizyolojisindeki rolünü araştırmayı amaçladık. Gastroenteroloji polikliniğine başvuran ve Roma III kriterlerine göre İBS tanısı konulmuş 54 hasta ile 34 sağlıklı birey çalışmaya dahil edildi. Çalışmamızda YKL-40 ve netrin-1 seviyeleri ELISA (Enzime Bağlı İmmunosorbent Analizi) yöntemiyle ölçüldü. Verilerin istatistiksel analizi için bağımsız örneklem t-testi kullanıldı. Çalışmamızda kontrol grubu ile karşılaştırıldığında, İBS hastalarında YKL-40 seviyelerinin anlamlı olarak değişmediğini bulduk. Netrin-1 seviyeleri ise kontrol grubuna göre İBS hastalarında anlamlı olarak düşük bulunmuştur. (p<0,001). Bulgularımız İBS hastalığının teşhis ve tedavisinde netrin-1’ in iyi bir belirteç olarak kullanılabileceğini YKL-40’ ın ise iyi bir belirteç olmadığını göstermektedir.
Irritable bowel syndrome is a common functional gastrointestinal disorder and adversely affects quality of life and leads to high health care costs. Pathophysiology of this disease is not known exactly but various hypotheses have been proposed. In many studies conducted recently, the role of low-level mucosal inflammation and immune system disfunction in the pathophysiology of IBS have been emphasized. It has been demonstrated that there is an imbalance in pro-inflammatory and anti-inflammatory cytokines in IBS patients. It has been suggested that YKL-40 protein which is secreted from various cells contributing inflammation and infection plays a role in immun regulation. Netrin-1 is a protein that has been shown to reduce inflammation mediated tissue injury and have anti-inflammatory effects. YKL-40 and netrin-1 have been thought to be associated with immun activation and inflammation and we aimed to investigate the role of YKL-40 and netrin-1 levels in IBS pathophysiology in our study. 54 patients who admitted to gastroenterology policinic and diagnosed as IBS according to Roma III criteria and 34 healthy individuals were included in the study. In our study, YKL-40 and netrin-1 levels were measured with ELISA. Independent samples t-test was used for statistical analysis. In our study, we found that YKL-40 levels in IBS patients did not change significantly compared to control group. Netrin-1 levels were found to be significantly lower in IBS patients compared to control group (p<0,001). Our findings demonstrate that YKL-40 is not a good marker to be used in diagnosis and treatment of IBS, however, netrin-1 is a good marker for this purpose.
