İzole ön çapraz bağ rekonstrüksiyonu ve ön çapraz bağ rekonstrüksiyonu ile birlikte menisküs cerrahileri yapılan hastaların klinik sonuçlarının karşılaştırılması

Özet
Ön çapraz bağ yırtıkları diz ekleminde özellikler futbol ve basketbol gibi sportif aktivitelerin, trafik ve iş kazaların sonucunda sıklıkla meydana gelebilen yaralanmalardır. Öte yandan çapraz bağ kopması ile birlikte menisküs yırtıkları eşlik edebilir. Özellikle aktivite düzey yüksek bireylerde veya sporcularda hayat kalitesini oldukça etkilenir. Retrospektif olan bu çalışmamızda izole ÖÇB rekonstrüksiyonu ve ÖÇB rekonstrüksiyonu ile birlikte menisküs tamiri veya menisküs rezeksiyonunun klinik sonuçlarını karşılaştırmak amaçlanmıştır. İzole ön çapraz bağ rekonstrüksiyonu grubunda 4 kadın ve 25 erkek, ÖÇB rekonstrüksiyonu ile birlikte menisküs tamiri yapılan grubunda 6 kadın ve 21 erkek, ÖÇB rekonstrüksiyon ile birlikte menisektomi yapılan grubunda 4'ü kadın 21'i erkek hasta dahil edildi. Tüm hastalarımız transtibial, dörtlü hamstring otogreft ile tek demet ve tek tünel endobutton yöntemiyle opere edildi. Menisküs yırtıkları ise tamamen içerde yöntemle onarıldı. Tüm grupların demografik özellikler, preop ve postop birinci yılda fonksiyonel skorlamaları (Lysholm, Tegner, Cicinnati ve İKDC), preop ve postop 1. yıl ön çekmece testi ve lachman testleri, yaralanma-operasyon arasında geçen süreleri, menisküsün yırtık tipi ve posterior tibial eğim açıları karşılaştırıldı. Menisküs rezeksiyonu yapılan grubun %56'si 12. aydan sonra ameliyat olup hastaların menisküs yırtıkların onarıma uygun olmayan bir menisküs yırtığı paternine karşılaşma olasılığı artmış olduğu görüldü. Her üç grubun post-op fonksiyonel sonuçları preop sonuçlarına göre anlamlı düzeyde yüksek ve her üç grubun erken dönemde klinik sonuçları tatmin edici seviyede saptadık. Tegner aktivite düzeyi skoru ön çapraz bağ rekonstrüksiyon ile beraber menisküs onarımı olan grupta menisküs rezeksiyonu yapılan grubuna göre anlamlı düzeyde yüksek saptadık.
Anterior cruciate ligament tears are injuries that can frequently occur in the knee joint as a result of sports activities such as football and basketball, traffic and work accidents. On the other hand, meniscus tears may accompany cruciate ligament rupture. It greatly affects the quality of life, especially in individuals or athletes with high activity levels. In this retrospective study, we aimed to compare the clinical results of isolated ACL reconstruction and meniscus repair or meniscus resection with ACL reconstruction. 4 female and 25 male patients were included in the isolated anterior cruciate ligament reconstruction group, 6 female and 21 male patients were included in the ACL reconstruction and meniscus repair group, and 4 female and 21 male patients were included in the ACL reconstruction and meniscectomy group. All our patients were operated on with transtibial, quadruple hamstring autograft and single-bundle and single-tunnel endobutton methods. Meniscus tears were repaired with all-inside technique. Demographic characteristics of all groups, preoperative and postoperative first year functional scores (Lysholm, Tegner, Cicinnati and IKDC), preoperative and postoperative first year anterior drawer test and Lachman tests, time between injury and operation, meniscus tear type and posterior tibial slope angles. compared. 56% of the meniscus resection group underwent surgery after the 12th month, and it was observed that the patients were more likely to encounter a meniscus tear pattern that was not suitable for repair. We found that the post-operative functional results of all three groups were significantly higher than the preoperative results, and the early clinical results of all three groups were satisfactory. We found that the Tegner activity level score was significantly higher in the group with meniscus repair combined with anterior cruciate ligament reconstruction than in the meniscus resection group.
