Özet
Araştırma, Koroner arter bypass greft ameliyatı olacak hastalarda ameliyat öncesi verilen rutin hasta eğitimi ile video destekli mobilizasyon eğitiminin ameliyat sonrası hareketlilik, kaygı düzeyi ve ağrı üzerine etkisini incelemek amacıyla randomize kontrollü bir çalışma olarak planlandı. Çalışma, Tekirdağ ilinde Şehir Hastanesi ve bir Devlet Hastanesinde 15.03.2023-15.09.2023 tarihleri arasında gerçekleştirildi. Koroner arter bypass greft ameliyatı olacak 40 hasta kontrol grubu, 40 hasta deney grubu olmak üzere toplam 80 hasta dahil edildi. Tüm hastalara ameliyat öncesi dönemde "Sürekli Kaygı Ölçeği" (SKÖ), "Durumluk Kaygı Ölçeği" (DKÖ), "Sosyo-demografik veri toplama formu" doldurtuldu. Kontrol grubuna rutin hasta eğitimi verilirken, deney grubuna ek olarak video destekli mobilizasyon eğitimi verildi. Tüm hastalara ameliyat olduktan sonra, ilk mobilizasyondan önce SKÖ ve DKÖ, ilk mobilizasyondan sonra DKÖ, "Hasta Hareketlilik Ölçeği" (HHÖ), "Gözlemci Hareketlilik Ölçeği" (GHÖ) doldurtuldu. 2. günde mobilizasyondan önce DKÖ, SKÖ, mobilizasyondan sonra DKÖ, HHÖ, GHÖ tekrar doldurtuldu. Gruplar arası karşılaştırmalar "Fisher testi" ve "Bağımsız Örneklem t Testi" ile incelendi. Tekrarlı ölçümlerin grup içi karşılaştırmaları ise "Tekrarlı ölçümler ANOVA testi" ile değerlendirildi. Bireylere ait hareketlilik, kaygı ve ağrı puanları arasındaki ilişkiler için "Pearson korelasyon testi" uygulandı. Deney grubundaki hastaların 2. gün mobilizasyon sonundaki durumluk kaygılarının diğer zamanlardan daha az olduğu bulundu. Deney grubundaki hastaların 1. mobilizasyondan itibaren durumluk kaygılarının kontrol grubundaki hastalara göre daha düşük olduğu belirlendi. Deney grubundaki hastaların kontrol grubuna göre 2. günde hareket aktivitelerinin arttığı bulundu.
The study was planned as a randomized controlled trial to investigate the effect of preoperative routine patient education and video-assisted mobilization training on postoperative mobility, anxiety level and pain in patients undergoing coronary artery bypass graft surgery. The study was conducted between 15.03.2023-15.09.2023 in the City Hospital and a State Hospital in Tekirdağ province. A total of 80 patients were included, 40 patients in the control group and 40 patients in the experimental group, who would undergo coronary artery bypass graft surgery. All patients were asked to fill out the "Trait Anxiety Scale" (SKÖ), "State Anxiety Scale" (DKÖ) and Socio-demographic data collection form in the preoperative period. While the control group received routine patient education, the experimental group received additional video-assisted mobilization training. After surgery, all patients were asked to fill out the SKÖ and DKÖ before the first mobilization, after the first mobilization, DKÖ, "Patient Mobility Scale" (HHÖ), and "Observer Mobility Scale" (GHÖ). On day 2, before mobilization, the DKÖ, SKÖ, and after mobilization, the DKÖ, HHÖ, and GHÖ were filled out again. Intergroup comparisons were analyzed by Fisher's test and Independent Sample t Test. Intragroup comparisons of repeated measurements were evaluated with Repeated measures ANOVA test. Pearson's correlation test was applied for the relationships between mobility, anxiety and pain scores of the individuals. he state anxiety of the patients in the experimental group at the end of mobilization on the 2nd day was found to be lower than at other times. It was determined that the state anxiety of the patients in the experimental group from the 1st mobilization was lower than the patients in the control group.It was found that patients in the experimental group had increased motor activity on day 2 compared to the control group.
