İnme geçiren hastalarda fonksiyonel bağımlılık durumunun depresyon ve yaşam kalitesine etkisi

Özet
Bu çalışma, inme geçiren hastaların fonksiyonel bağımlılık durumlarının, depresyon ve yaşam kalitesine etkisini değerlendirmek amacıyla tanımlayıcı nitelikte planlandı. Nisan 2018-Mayıs 2019 tarihleri arasında Enstitüler Sağlık Uygulama ve Araştırma Merkezi Fizik Tedavi ve Rehabilitasyon Servisi, Ayaktan Tedavi Ünitesi ve Polikliniği'nde yapıldı. Araştırmanın örneklemini, 18 yaş ve üzerinde inme tanısı almış çalışmaya katılmayı gönüllü olarak kabul eden 125 hasta oluşturdu. Veriler; araştırmacı tarafından, 'Hasta Veri Toplama Formu', 'Beck Depresyon Envanteri', 'Fonksiyonel Bağımsızlık Ölçeği', 'Barthel İndeksi' ve 'İnme Etki Ölçeği 3.0', formları kullanılarak yüz yüze görüşme yöntemi ile elde edildi. Verilerin değerlendirilmesinde, ortalama, standart sapma, frekans, yüzde, Pearson korelasyon testi, bağımsız gruplar t testi, tek yönlü varyans analizi (ANOVA) kullanıldı. Sonuçlar %95'lik güven aralığında ve p<0,05 anlamlılık düzeyinde değerlendirildi. Hastaların yaş ortalamaları 57,14±14,05 olup, %61,6'sı erkek, %56'sı ilköğretim mezunudur. Hastaların, %51,2'si 2-5 yıldır inme hastası olup, çoğunluğunun (%74,4) inme tipi iskemik inmedir. Hastaların; Fonksiyonel Bağımsızlık Ölçeği toplam ortalama puanı 77,04±19,44 olup, %80,8'inin yarı bağımlı olduğu, Barthel İndeks toplam ortalama puanı 46,36±23,4 olup, %59,2'sinin ileri derecede bağımlı olduğu belirlenmiştir. Beck Depresyon Ölçeği ortalama puanı 21,2±10 olup, %55,2'sinde depresif belirti bulunduğu ve İnme Etki Ölçeği 3,0 toplam ortalama puanı 169,21±27,28 olduğu ve yaşam kalitelerinin olumsuz yönde etkilendiği saptanmıştır. Çalışma sonucunda; hastaların fonksiyonel bağımsızlık düzeyleri arttıkça yaşam kalitelerininde arttığı saptanmıştır (p<0,001). Fonksiyonel bağımlılık durumunun artmasının ise depresif belirti düzeylerinin artmasına yol açtığı belirlenmiştir (p<0,001). Hastaların depresif belirti durumları ile yaşam kaliteleri arasında ise negatif yönlü istatistiksel açıdan ileri derecede anlamlı ilişki olduğu saptanmıştır (p<0,001). İnmeli hastaların fonksiyonel bağımlılık durumlarının yaşam kalitesini ve depresif belirtileri etkilediği belirlenmiştir.
This study was planned having descriptive quality to evaluate the effect of the functional dependence situation of the patients who had a stroke on depression and quality of life. It was implemented at Tekirdağ Namık Kemal University, Health Practice and Search Centre, Physiotherapy and Rehabilitation Service, Outpatient Clinic between April 2018 and May 2019. The sample of the study consisted of 125 patients aged 18 years and over with a diagnosis of stroke and voluntarily accepted to participate in the study. The data was obtained by the researcher through face to face interview method by using 'Patient Data Collection Form', 'Beck Depression Inventory', 'Functional Independence Scale', 'Barthel Index' and 'Stroke Effect Scale 3.0' forms. On the evaluation of the data, average standard deviation, frequency, percentage, Pearson correlation test, independent groups t test, track one-way analiysis of variance (ANOVA) were used. Results were evaluated in 95% confidence interval and p<0,05 significance level. Age average of the patients is 57.14±14.05 and 61.6% of the mismale and 56% is primary school graduate. 51.2% of the patients has been suffering from the stroke for 2-5 years, most of them (74.4%) have ischemic stroke. Total average point of "Functional Independence Scale" of the patients is 77.04±19.44 , 80.8% is semi dependent, Barthel Index total average point is 46.36±23.4, 59.2% is dependent on highest degree. Total average point of Beck Depression Inventory is 21.2±10, 55.2% of them have depressive symptoms, total average point of Stroke Effect Scale 3.0 is 169.21±27.28 and it was detected that their quality of life was affected negatively. As a result of the research; it was determined that as the functional independence level of the patients increases, their quality of life increases as well (p<0,001). It was indicated that the increase in the state of functional dependence causes the increase on the levels of depressive symptoms (p<0,001). It has been determined that there is statistically a negative correlation on the highest degree between the state of depressive symptoms of the patients and quality of their life (p <0.001). It has been designated that the state of functional dependence of stroke patients affects quality of life and depressive symptoms.
