Özet
Meme kanseri dünyada olduğu gibi ülkemizde de tüm kadın kanserleri içinde en sık olarak gözlenen kanserdir. Bağışıklı sisteminin hastalık sürecindeki prognostik rolü son yıllarda giderek önem kazanmaktadır. Kanser hastalarında tümör ve bağışıklık sistemi arasındaki etkileşimi değerlendirme için periferik kandaki T lenfositlerin alt gruplarından faydalanılabilir. Bizde erken evre meme kanserinde periferik kanda T lenfosit alt gruplarının durumunu değerlendirmeyi amaçladık. Bu nedenle, neoadjuvan kemoterapi alan 47 meme kanserli olgunun başlangıç periferik T lenfosit alt grupları ile tedavi yanıtı (en iyi objektif yanıt-EIOY) arasındaki ilişkiyiaraştırtık. Çalışmamızdaolguların başlangıç CD3, CD4 ve CD8 mutlak sayısı ile EİOY arasında negatif yönde anlamlı bir ilişki saptandı. Ayrıca EİOY'ı etkileyen diğer faktörler incelediğimizde CD4/PD1 ekspresyonu yüksek olan hastalarda, tedavi yanıtı kötü olarak bulundu (OR:0.212 %95 CI: 0,061-0,735, p=0.015). Benzer şekilde CD8/PD1 expresyonu yüksek olanlarda objektif yanıt oranı anlamlı olarak düşük saptandı. (OR: 0,250 %95 CI: 0,073-0,858, p=0,028). Sonuç olarak, meme kanserinde periferik kandaki T lenfositlerin alt grup dağılımları hem tedavi yanıtına hem de hastalıkla ile ilişkili sağkalıma etkisi olabileceği gösterilmiştir.
Breast cancer is the most frequently observed cancer among all female cancers in both the world and in our country. The prognostic role of the immune system in the disease process has become increasingly important in recent years. T lymphocyte subgroups in peripheral blood can be used to evaluate the interaction between tumor and immune system in cancer patients. We aimed to evaluate the status of T lymphocyte subsets in peripheral blood in early stage breast cancer. Forthisreason, we evaluated the relationship between baseline peripheral T lymphocyte subsets and treatment response (best objective response) in 47 breast cancer patients receiving neoadjuvant chemotherapy. In our study, we found a significant negative relationship between baseline CD3, CD4 and CD8 absolute counts and best objective response. We also found that the best objective response the other factors were evaluated, as a result of this analysis, treatment response was found to be poor in patients with high CD4/PD1 expression (OR: 0.212, 95% CI: 0.061-0.735, p=0.015). Similarly, the objective response rate was significantly lower in patients with high CD8/PD1 expression (OR: 0.250, 95% CI: 0.073-0.858, p=0.028). In conclusion, we suggest that the subgroup distribution of T lymphocytes in peripheral blood may have an effect on treatment response in breast cancer.
