Öğe Dietary modifications and supplementation in breastfeeding mothers(Springer International Publishing, 2023) Aslan, Ilke Özer; Doğan, KezibanBreastfeeding mothers usually feel torn between the need to diet in order to lose excess weight gained during pregnancy and the need to maintain an adequate dietary intake to ensure milk is available for the infant. Lactation involves a complex interaction of different factors, including nutrition, endocrine effects and the mother's behaviour. Recently there have accumulated important insights into how the nutritional status of the mother influences breastfeeding and the infant's healthy development. Nonetheless, much is still unknown regarding the necessary dietary intake of trace elements in breastfeeding women. Some studies have examined how dietary supplementation with zinc, copper or iodine affects the level of these ions in human milk. In addition, the presence of comorbidities such as obesity, diabetes mellitus, dyslipidemia, hypertension, polycystic ovary syndrome and principles behind dietary prescriptions are also very effective factors in breastfeeding. Management of diet should proceed stepwise. The important factors to consider in recommending a suitable diet are the individual's basal metabolic requirements, the balance of macro- and micro-nutrients needed and the objectives aimed for. If dietary management is approached from a realistic, personalised perspective, and any prescribed diet is provided in a patient-friendly format, weight loss can be achieved in women following pregnancy and in middle age. Whatever diet is followed, however, one constant feature is that all breastfeeding mothers will require additional fluid intake and a varied diet to ensure milk production is sufficient and of high quality. © Springer Nature Switzerland AG 2023. All rights reserved.Öğe Management of preterm labor and birth(Nova Science Publishers, Inc., 2023) Aslan, Ilke ÖzerPreterm births are the leading cause of neonatal morbidity and mortality and the most common cause of prenatal hospitalization. Approximately 12% of live births worldwide occur in the preterm period, and preterm labor leads to 50% of preterm births. Although it is known that some factors increase the risk associated with preterm labor, it is not known exactly which patients will experience preterm labor. In suitable patients with a history of preterm birth, prophylactic cerclage or vaginal progesterone treatment are alternatives that should be kept in mind. The aim should be to select the most appropriate tocolytic therapy in the appropriate patient, to organize lung maturation with antenatal corticosteroid therapy and neuroprotective therapy with magnesium sulfate therapy, to ensure that delivery is carried out in the most appropriate center for the neonatal intensive care unit under the most appropriate conditions. © 2023 Nova Science Publishers, Inc. All rights reserved.Öğe The management of pelvic organ prolapse(Nova Science Publishers, Inc., 2024) Aslan, Ilke ÖzerPelvic organ prolapse can be defined as the collapse of structures in the pelvic region downwards towards the vaginal region after the disruption of their anatomical support. It is a very common condition, affecting up to 50% of postmenopausal cases, especially in older ages. It significantly negatively affects the quality of life by causing a feeling of pressure in the pelvic area, urinary and bowel dysfunction, and sexual dysfunction. The management of pelvic organ prolapse should begin with an accurate and detailed evaluation. Different surgical and non-surgical procedures stand out in management, starting from conservative treatments. In the management of pelvic organ prolapse cases, an individualized treatment plan must be planned, and an individual road map must be determined for each patient. © 2024 Nova Science Publishers, Inc.
