Türkiye’ de sağlık hizmetlerinde piyasalaşma süreci sağlıkta dönüşüm programı kapsamında kamu özel ortaklığı modeli

Abstract

Sağlık ve sağlık hizmetleri, 19.yüzyılın sonlarında ortaya çıkan sosyal devlet anlayışının 20. yüzyılının sonlarına doğru genişlemesiyle insanların sahibi olduğu sosyal bir hak haline gelmiştir. Sağlığın sosyal yönü devletin sağlık sektörü içinde çeşitli rollerde var olmasını gerektirmektedir. Bu çalışmada, Türkiye’nin kuruluş yıllarından itibaren uygulanan sağlık politikaları devlet anlayışları ekseninde ele alarak nasıl değişim gösterdiği ve devletin sağlık alanında nerede ve nasıl rol aldığı dönemsel olarak incelenmiştir. Türkiye’de Kamu özel ortaklığı ile uygulanan yapişlet, yap-işlet-devret ve yap-kirala modelleri ile yapılan entegre şehir hastanelerine geçiş boyunca sağlık alanında meydana gelen birçok tahavvül teşhir edilmiştir. Kamu-Özel Ortaklığı (KÖO) yöntemi kamu ile özel sektörün sağlık alanındaki ortaklığına dair olarak Türk sağlık sektörü içerisinde de tatbik etmeye başlanılmıştır. Türkiye son yıllarda sağlık sektörü içerisinde geniş çaplı varyasyonlar yaparak KÖO’yu artırmaktadır. Bu doğrultuda çalışma Türkiye’nin sağlık sektörüne ilişkin politikalarında oluşan bu varyasyona odaklanmıştır. Çalışmanın sonuçları doğrultusunda Kamu Özel Ortaklığı Modeli (KÖOM) ile sağlık tesislerinin yapılması, atıl sermayeler ile toplum yararına yeni yatırımlar oluşturulmasına, daha kısa zamanda sonuçlanan etkin ve kaliteli kamu hizmetleri sunumunun sağlamasına, kamunun bütçe sıkıntısından kaynaklı gerçekleştirilemeyen yatırımların oluşturulmasına katkı sağlayacağı düşünülmektedir.

Health and health services have become a social right for the people with the expansion of social state understanding, which emerged at the end of the 19th century, towards the end of the 20th century. The social aspect of health requires the state to exist in various roles within the scope of the health sector. This studyevaluated the health policies implemented since the founding years of Turkey in theaxis of state understandings and periodically examined how they have changed andalso where and how the state played a role in the field of health. Manytransformations that occurred in the field of health during the shift to the cityhospitals that have been made with build-operate, build-operate-transfer and buildlease models that have been implemented under the umbrella of Public-privatepartnership have been demonstrated. The Public-Private Partnership (PPP) methodhas started to be implemented in the Turkish health sector regarding the partnershipbetween the public and private sectors in the field of health. Turkey has been increasing the PPP by making large-scale variations in the health sector in recentyears. In this direction, the study focuses on those variations that occur in the policiesregarding Turkey's health sector. In line with the results of the study, it is thoughtthat the erection of health facilities with the Public-Private Partnership Model (PPPM) will contribute to the creation of new investments for the benefit of thesociety, the providing of effective and high quality public services within a shorter period of time and to the creation of investments that cannot be realized due tobudgetary problems of the public with the utilization of idle capital.
