Diyabetik bireylerde bakım gereksinimlerinin yaşam kalitesi ve kronik hastalık bakımına etkisi

Abstract

Bu çalışmayla; diyabetik bireylerde bakım gereksinimlerinin yaşam kalitesi ve kronik hastalık bakımına etkisinin incelenmesi amaçlandı. Tekirdağ Namık Kemal Üniversitesi Sağlık Uygulama ve Araştırma Merkezi Endokrinoloji ve Metabolizma Hastalıkları Polikliniği‘nde Ocak- Haziran 2017 tarihleri arasında tip 2 diyabet tanısıyla takip edilen hastalar arasından, çalı?maya katılmaya gönüllü olanlar araştırmanın örneklemini (n=150) oluşturdu. Verilerin toplanmasında ?Hasta Tanıtım ve Bakım Yönetimi Değerlendirme Formu?, ?Yaşam Kalitesi Değerlendirme Ölçeği (EQ-5D-5L)? ve ?Kronik Hastalık Bakımını Değerlendirme Ölçeği-Hasta Formu (PACIC)? kullanıldı. Formlar araştırmacı tarafından yüz yüze görüşme tekniği ile dolduruldu. Verilerin değerlendirilmesinde Oneway Anova test ve Tukey HDS test, bağımsız iki grup t testi, Kruskal Wallis testi, Mann Whitney U testi ve Pearson korelasyon analizi kullanıldı. Çalışmaya katılan diyabetlilerin kronik hastalık yönetiminin iyi olduğu, PACIC skoru toplam puan ortalamasının 3,78±0,79 ve EQ-5D-5L genel sağlık skoru ortalama puanının ise 0,84±0,17 değeriyle kusursuz sağlığa yakın olduğu saptandı. Diyabetlilerin EQ-5D-5L indeks puanları ile PACIC skoru karar verme desteği ve amaç belirleme/rehberlik alt boyut puanları arasında pozitif yönde anlamlı ilişki, beden kitle indeksi ve HbA1c değerleri ile EQ-5D-5L genel sağlık skoru arasında ise negatif yönde anlamlı ilişki saptandı. Diyabetlilerin şu anki diyabet tedavi uyumu ile EQ-5D-5L genel sağlık skoru ve PACIC skoru toplam puanı arasında anlamlı ilişki olduğu saptandı. Diyabetlilerin diyabet eğitimi alma durumuna göre PACIC skoru karar verme desteği ve amaç belirleme/rehberlik alt boyut puanları bakımından anlamlı fark olduğu görüldü. Araştırma sonucunda, diyabetlilerin kronik hastalık bakım yönetiminde; genel tedaviye uyumu için sağlık eğitimi ve danışmanlık programları planlanması önerilebilir.

The aim of this study was to investigate the effect of care needs on quality of life and chronic disease care in patients with type-2 diabetes. One hundred forty-five patients with type-2 diabetes who were followed-up between January 2016 and January 2017 in Namık Kemal University Training and Research hospital, Endocrinology and Metabolism outpatient clinic, were included in the study. The "Patient Presentation and Care Management Evaluation Form", "Quality of Life Assessment Scale (EQ-5D-5L)" and "Chronic Illness Care Assessment Scale-Patient Form (PACIC)" were used for the collection of data. The forms were filled by face-to-face interview technique carried out by the researcher. Oneway Anova test, Tukey HDS test, independent two sample t test, Kruskal Wallis test, Mann Whitney U test and Pearson correlation analysis were used for the evaluation of the data. In this study, the average total PACIC score of patients with with type-2 diabetes was found to be satisfactory (3,78±0,79). However, the EQ-5D-5L average score was found to be close to perfect (0,84 ± 0,17). A positive correlation was found between EQ-5D-5L scores and PACIC scores‘ decision support and goal setting / guidance sub-scale scores, and a negative correlation was found between positive EQ-5D-5L score and body mass index and HbA1c values. A significant correlation was also found between the current diabetes treatment compliance and the EQ-5D-5L general health score and PACIC total score. The PACIC scores’ decision support and goal setting/guidance sub-scale was found to be significantly different according to the diabetes education status of the patients with type-2 diabetes. As a result of the research, planning health education and counseling programs for general care adjustment may be recommended for chronic disease management of diabetes.
