FETAL HAREKETLERDE AZALMA ŞİKAYETİ OLAN GEBELERİN PERİNATAL DEĞERLENDİRİLMESİ
Özet
Amaç: Klinisyenlerin antenatal fetal hareketlerde azalma gördükleri gebelerde, fetal kayıp olabileceği için tedirgin olup olmamaları gerektiğine, perinatal sonuçlar değerlendirilerek farkındalık yaratmak amaçlanmıştır. Materyal ve Metot: Çalışmamıza Eylül 2017 ile Eylül 2018 tarihleri arasında hastanemize fetal hareketlerde azalma veya fetal hareketleri hissetmeme şikayeti ile başvuran 32 hafta ve üzeri gebelik yaşı olan 105 gebe dahil edilmiştir. Fetal hareketlerde azalmaya sebep olabilecek hastalığı olanlar, bu duruma sebep olabilecek ilaç kullanımı olanlar ile çoğul gebeliği olanlar çalışma dışı tutulmuştur. Çalışmaya katılan 105 gebenin yaş, gravida, parite, abortus, küretaj öyküleri ve eğitim düzeyleri kayıt altına alındı. 105 gebenin perinatal sonuçları açısından sezaryen endikasyonları, doppler ultrasonografi sonuçları kayıt edilmiştir. Bulgular: Fetal hareketlerde azalma ön tanısıyla hastaneye yatışı yapılan gebelerin 80 tanesi (%76) gerekli tetkikler uygulandıktan sonra, takipleri sırasında normal bulgular izlenip taburcu edilmişlerdir. Taburcu edilenlerin 64 tanesinde (%60) gebeler yalnızca hidrate edilmiş ve non stres test (NST) takipleri yapılmıştır. Bu gebeler başka bir tedaviye ihtiyaç duymadan taburcu olmuşlardır. 105 hastadan 25 tanesinde (%23) fetal stres düşünülüp sezaryen(c/s) yapılmıştır. Fetal hareketlerde azalma şikayeti ile hastanemize başvuran gebelerden 4 tanesinde (%3,8) intrauterin exitus izlenmiş olup aynı zaman diliminde hastaneye yatan diğer hastalardan ise 1 tanesinde (%0,04) intrauterin exitus izlenmiştir. Sonuç: Fetal hareketlerde azalma şikayeti ile başvuran hastalarda sezaryen oranları ve ani fetal ölüm oranları artmıştır. Bu şikayet ile gelen hastaların yönetimi klinisyenler için zorluklar içermektedir. Fetal hareket sayımı konusunda bir fikir birliğinin bulunmaması nedeni ile halen bu vakalarda yaklaşımda kullanılabilecek bir algoritma oluşturulamamıştır. Age, gravida, parity, abortion, curettage and education levels of 105 pregnant women were recorded. Perception of the perinatal outcomes of 105 pregnant women, doppler ultrasonography results were recorded. Results: Among the study group, 80 of these pregnant women (%76) who were interned with the prediagnosis of fetal movements had normal findings during their follow-up and were discharged. In 64 (60%) patients who were discharged, only hydration treatment was given and non stress test (NST) results were followed without any further treatment. 25 out of 105 patients (23%) underwent fetal stres and had cesarean section (c / s). Intrauterine exitus was observed in 4 (3.8%) pregnant women who applied to our hospital with the complaint of decrease in fetal movements. İntrauterine exitus was observed in 1 (0.04%) of the other hospitalized patients during the same period of time. Conclusions: Cesarean rates and sudden fetal mortality rates are found to be increased in patients presenting with a decrease in fetal movements. Patients presenting with this condition puts the attending physician in a difficult situation. Due to the lack of consensus on the methods of fetal motion count, an algorithm for the management of decreased fetal movement has still not been established.
