Double RCA variations are incidentally discovered, because they are asymptomatic. The differentiation of true double RCA and high take off of a large right ventricular branch (RVB) in coronary angiograms is a condition that requires quite remarkable attention. Here, we reported a case with acute inferior myocard infarction in whom high take off of a large RVB originated from proximal RCA, mimicking a double RCA.
