Avrupa Birliği üyesi ülkelerin sağlık sistemi ile Türk Sağlık sisteminin karşılaştırılması: Türkiye Almanya örneği

Abstract

Türkiye uzun yıllardır Avrupa Birliği’ne girmek için çaba sarf etmektedir. Zaman zaman ilişkilerde inişler ve çıkışlar görülse de son yıllarda genellikle olumsuz gelişmelerin yaşandığı söylenebilir. Bu ortamda Türkiye’nin AB’ye girme konusunda pek heyecanlı olduğu söylenemez. AB’ye üye olunamasa da AB standartlarında bir yaşama ulaşılması gerektiği herkesin ortak kanaatidir. Hâlihazırda AB üye olacak devletlerden bazı kriterleri yerine getirmesini beklemektedir. Sağlık ise AB’nin ortak bir sisteme sahip olmadığı alanlardan biridir. Üye ülkelerin kendileri sağlık hizmetlerinin sunumu, yönetimi ve finansmanı konusunda esas belirleyicidir. Bu nedenle Türkiye ve AB arasında sağlık alanında bir karşılaştırma yapabilmek ve eksikleri tespit edebilmek adına örnek bir ülke seçilmesi daha uygun olacaktır. Bu nedenle de AB’nin en önemli ülkelerinden biri olan ve nüfusu bakımından Türkiye’ye yakın olan Almanya bu çalışmada konu edinilmiştir. Türkiye ve Almanya sağlık sektörünün işleyişi açısından pek çok benzer noktaya sahiptir; nüfusun tamamını kapsayan bir sigorta sistemi, yakın oranlarda seyreden kamu finansmanı, sağlık hizmetlerinin kaç basamakta sunulduğu vs. Ancak Almanya gelişmiş ekonomisi sebebiyle pek çok kalemde Türkiye’nin önünde yer almaktadır. Hastane sayısı, hastanelerdeki yatak sayısı, doktor- diş hekimi-eczacı sayısı, MR- BT gibi teknolojik cihaz sayısı göz önüne alındığında Türkiye, Almanya’nın çok gerisinde kalmaktadır. Bu çalışmada en güncel veriler ve resmi istatistikler kullanılarak farklılıklar ortaya konulmuştur. Pek çok alanda olduğu gibi sağlık sektöründe de uçurumların ortadan kalkmasının ilk şartı yeterli finansmanın sağlanmasıdır.

Turkey is striving to enter the European Union for many years. Although there are ups and downs in relations from time to time, it can be said that there have been negative developments in recent years. In this environment, on Turkey's EU membership is not very exciting. It is a common opinion that everyone should reach a life in EU standards even if they do not become an EU member. Currently, EU member states are expected to meet certain criteria. Health is one of the areas where the EU does not have a common system. Member States themselves are the main determinants of the provision, management and financing of health services. Therefore, in order to make a comparison example in the field of health between Turkey and the EU it would be more appropriate to select a country. For this reason, Germany, which is one of the most important countries of the EU and close to Turkey in terms of population, is the subject of this study. Turkey and has many points of similarity in terms of the functioning of the health sector in Germany; an insurance system that covers the entire population, public funding at close rates, the number of steps in which health services are provided, and so on. However, due to the advanced economies, Germany is located in front of Turkey in many areas. Considering the number of hospitals, number of beds in hospitals, number of doctor-dentist-pharmacist, number of technological devices such as MRI- CT, Turkey lags far behind Germany. In this study, differenceswere revealed by using the most up-to-date data and official statistics. As in many areas, the first condition for the abolition of gaps in the health sector is the provisionof adequate financing.
